Long Term Care Services

If you or a loved one is an elder, has a physical disability, or chronic disease, we can help you stay in your homes for as long as possible by providing personal care and support services through our Long Term Care Programs. Our Long Term Care programs and services include:

  • Homemaker
  • Choices for Care
  • Interview with grateful family about their long term care experience.

Homemaker

Homemakers can assist you with light housekeeping, shopping, errands, laundry, meal preparation, and other necessary tasks to help you remain at home. The Homemaker program supports disabled and/or elderly individuals. Funding for the Homemaker program comes from the Franklin-Grand Isle United Way and state support.

Choices for Care

Long term care services are available to you as an alternative to nursing homes through State of Vermont funding. Services available include case management, chore services, and assistance with personal care including bathing, grooming, toileting, transfers and ambulation. Other services include adult day care and assistive technology.
