Chronic Disease Management

Every year, chronic diseases such as heart disease, cancer and diabetes are responsible for millions of premature deaths. Americans miss 2.5 billion days of work because of these illnesses, and all of that lost productivity adds up to more than $1 trillion, according to the Robert Wood Johnson Foundation. We want to help counteract this trend by helping you learn how to manage your condition on your own.

Through our Chronic Disease Management program, your doctor can make a referral to our agency for care from our Home Care Nurses. Our nurse will visit you after you are discharged from the hospital to teach you how to manage your condition and give you valuable tools to manage your health in order to prevent unnecessary readmission to the hospital.
