& Hospice Month
We would like to take the opportunity during
National Home Care & Hospice Month to share some of
our achievements.
For over 38 years, Franklin County Home Health
Agency has provided exceptional home and community based health care to the
residents of Franklin County.
We take pride in bringing medically
necessary home health care wherever it is needed, regardless of a
client’s ability to pay, location of residence or complexity of health
issues.
Our programs span a lifetime – from prenatal care to
care for young families to rehabilitation, long term care, and end-of-life care.
Franklin County Home Health Agency has kept pace
with the latest healthcare trends and technologies. In the face of shrinking
reimbursements, we continue to identify community needs and provide essential
cost-effective healthcare services to some of our most vulnerable citizens.
During fiscal year 2007,
FCHHA:

provided almost
96,000 visits to 1600 different patients.

administered
just under 1400 flu vaccinations in our public clinics.

 |
hospice
volunteers provided 1,136 hours of support to terminally ill
patients and their families.

received a
Silver Award for Worksite Wellness from the
Vermont Governor’s Council on
Physical Fitness and Sports.

 |
fully
implemented a state-of-the-art telemonitoring program.
|
 |
reduced
the rate of hospitalization experienced by our
patients.

worked
diligently to streamline operations to become as effective and as efficient as
possible. |
