Long Term Care

Our Long Term Care Programs provide personal care and support services for elders and people with physical disabilities or chronic diseases.

Long Term Care Programs and Services include:

Choices for Care
Through state funding, long term care services are available top you as an alternative to nursing homes. Services available include case management, chore services, and assistance with personal care including bathing, grooming, toileting, transfers and ambulation.  Other services include adult day care and assistive technology.  

Homemaker

Homemakers can assist you with light housekeeping, shopping, errands, laundry, meal preparation, and other necessary tasks to help you remain at home.  The Homemaker program supports disabled and/or elderly individuals.  Funding for the Homemaker program comes from United Way and state support.

CareNet, Private Duty
Services are offered through CareNet on a private pay basis.  Capable health care personnel offer assistance with personal care, chores, homemaking and respite.
