FCHHA is a Medicare Certified Hospice provider.
Hospice provides quality compassionate care at the end of life. 

The Hospice team provides expert medical care, pain management and emotional and spiritual support tailored to each patient's needs and wishes. 

In addition, Hospice offers invaluable support, caregiving training and assistance to family members. 

The following services are available through Hospice:

• Bereavement Support

It is important to note that Hospice is not designed to provide 24 hour care.  A primary caregiver is an important part of the team, providing care to the patient when the patient is no longer safe alone.

Palliative Care

Palliative Care provides expert pain management and
comfort care if you have a chronic illness, uncontrolled pain
or a life-threatening illness.
