Home Care

The Home Care Team is made of many people working together to provide you the best care.  We create a plan of care with you and your physician to respond to your health care needs in the comfort of your home. 

To promote recovery, you and your family are taught to manage your illness and improve your independence.

We also offer a home telemonitoring program which allows us to regularly monitor your condition state-of-the-art technology.

The Home Care team begins with you.
You and your family are the most important members of the team.  We encourage your participation in planning care to meet your needs. 

Our staff will work closely with you and your physician to develop a home based plan of care that takes into consideration your mental, physical and emotional status.

Your care is delivered by a team of qualified and caring professionals.  This team may include:

  • Skilled Nurses

  • Medical Social Workers

  • Occupational Therapists

  • Physical Therapists

  • Speech and Language Pathologists

  • Licensed Nursing Assistants and Care Attendants
