Who may benefit from home care?

People of any age with medically necessary home health care needs may benefit from home health care.  This might include:

  • people recently discharged from the hospital or nursing home in need of continued care,

  • people receiving treatment for cancer and other chronic illnesses,

  • expectant mothers preparing for childbirth and care for their newborns,

  • children in need of physical, occupational or speech therapy,

  • terminally ill people who want to be cared for at home with compassion and dignity, and support for their families

We provide services throughout the 15 cities and towns of  Franklin County, Vermont.

How do I begin receiving care?
Patients can be referred from a physician, a hospital, nursing home, family, friends or a self-referral.  

Please call (802) 527-7531 and ask to speak with our intake coordinator.  Our intake coordinator will be happy to assist you in arranging for home care services.  We can also make a call to your physician to help assess your home healthcare needs

How do patients pay for home care services?
Payment for home care comes from many sources including Medicare, Medicaid and private insurance.  We provide care to anyone in need of medically necessary home care, regardless of their ability to pay.  This is made possible by the generosity of the towns we serve, businesses and individual donors.

How do I make a gift to FCHHA?
To make a monetary donation, you may mail a check or money order to FCHHA, 3 Home Health Circle, St. Albans, VT  05478.

You may also call us at (802) 527-7531 to make a gift with your Visa or MasterCard.

Franklin County Home Health Agency is a charitable organization under section 501(c)3 of the Internal Revenue Service code and contributions are tax-deductible to the fullest extent allowed by law. 

Gifts may be made in a single payment, pledged and fulfilled over a year or a period of years, or may be made as an estate gift.

Memorial gifts are a wonderful way to honor a loved one.  You may also wish to make a gift in honor of someone special to recognize a special occasion.  We will notify the family or individuals of your generosity.

How can I volunteer to help FCHHA?
There are many ways to support FCHHA.  You can train to be a hospice volunteer, become a sponsor for one of our events or participate in a fundraising event.
