Arranging for Home Health Care
When
you call our office, you will speak with a nurse
who will listen
and
help you determine your home healthcare needs.
Please call (802) 527-7531
and ask to speak with
our intake coordinator.
Referrals may come from physicians, discharge planners, other health care
providers, your family or friends, or you.
Our intake coordinator will be happy to assist you in
arranging for home care
services. We can also make a
call to your physician to help assess your
home healthcare needs.
Home Care Can Help…

if you have been
recently discharged from the hospital or nursing home and are in need of
continued care,

if you are receiving treatment for cancer and other chronic
illnesses,

if you are an expectant mother preparing for childbirth and care for your newborn,
-
if
your child needs physical, occupational or
speech therapy,
-
if you have a terminal illness and choose to be cared for at home with compassion and
dignity, and support for your family.
Financial Arrangements
Payment for home care comes from many sources
including Medicare, Medicaid and private insurance. We provide care
regardless of your ability to pay if you are in need of medically necessary home care. This is made possible by the generosity of the towns we serve,
our local United Way, businesses and individual
donors.
