Our Long Term Care Programs provide personal care and support services for elders and people with physical disabilities or chronic diseases. 

Long Term Care Programs and Services include:

bulletChoices for Care
Through state funding, long term care services are available as an alternative to nursing homes. In addition to the homemaker services, clients receive case management, and simple personal care assistance with bathing, grooming, toileting, transfers and ambulation. Also available is assistance with obtaining necessary medical equipment and emergency response systems to promote independence.
 
bullet Homemaker
Homemakers help patients with light housekeeping, shopping, errands, laundry, meal preparation, and other necessary tasks to help the patient remain at home.  The Homemaker program supports disabled and/or elderly individuals.  Patients must meet criteria for sliding scale state grant.
 
bullet CareNet (click here to learn more)
Private Duty services are offered through CareNet on a private pay basis.  Capable health care personnel offer assistance with personal care, chores, homemaking and respite.
