Gifts may be made in a single payment,
pledged and fulfilled over a year or a period
of years, or may be made as an estate gift.

with your contribution:

	Name of donor(s)
	Donor's address to where acknowledgement should be sent
	Preferred designation if applicable
	Name of person being honored or remembered if applicable
	Name and address of individual to be advised of an honorary or memorial gift (the amount of your donation remains confidential)
