Tell us what you'd like to know ...

... about your end of life care options.

The Vermont Ethics Network, in partnership with the Palliative Care and Pain Management Task Force and the Madison-Deane Initiative, has put together
a survey to measure understanding of Palliative and End of Life Care. They also want to learn about people’s related educational needs. Your responses will help
in organizing future educational opportunities within your community and across Vermont. Thank you for taking the time to complete this survey, which has 15 questions and should take less than 5 minutes to complete. Contact the Vermont Ethics Network with any questions or comments:
Email: ven@vtethicsnetwork.org, or call (802) 828-2909.
